Based on the evidence obtained during the DFPS investigation, there is sufficient evidence to support that an eight-year-old child was left in a vehicle unattended.
Based on the evidence obtained during the DFPS investigation, there is sufficient evidence to support that the driver failed to account for all children before leaving the vehicle unattended.
This standard was found deficient as part of a DFPS Investigation. There is sufficient evidence to support the allegation of neglectful supervision. A caregiver did not check the van after transporting, which resulted in an eight-year-old child being left in a vehicle unattended for approximately 13 minutes.
During the DFPS Investigation, it was found a child was left unsupervised on the van. The caregiver at the time did not do a face to name check when all the children left the bus.
A review of the Employees Who Left Employment in 2024 Survey data from January 17, 2025, revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.
A child medication expired November 2024.
Based on interviews conducted during the investigation, the parent were not notified at the time of pick of the incident.
I observed that several bottles of medication , albuterol(3), cream, motrin, and dexmethyphenidate, not labeled with the date it was brought into the operation. Photos were taken.
