During the inspection the director reported that two staff failed to supervise properly and left a child behind in the classroom.

One staff members missing one hour of child abuse and neglect training.

The operation had not submitted the total number of employees who ceased working at the operation the previous calendar year by January 15, 2024.

Based on the information obtained during investigation, it was determined a caregiver used inappropriate discipline when she hit a child on the hand.

There was a small playground equipment with opening and cracks with exposed sharp edges. Note: equipment removed during inspection.

The children's records evaluated were missing authorization for emergency medical care.

During inspection on 09/15/22, several classrooms were closed due to mold in the building and this was not reported to Child Care Regulations.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

Based on information obtained during the investigation, there was a lack of supervision while the class was transitioning from the playground to the classroom.
