The operations last fire inspection was in October 2024. The operation will contact the fire marshal to schedule a new inspection.

During a DFPS investigation it was found that a caregiver laid their arm on a child to get the child to sleep.

During a DFPS investigation, it was found that a child was injured and the parents of the child were not notified of the injury by operation staff.

A caregiver did not use good judgement when the caregiver pulled a child's leg while the child was on their mat, making the child hit their head on a shelf.

Observed 2 staff files with 24 hours of training but were missing some required trainings to include; 1.Administering medication 2.Preventing and responding to emergencies due to food or an allergic reaction 3.Administering medication 4.Handling, storing, and disposing of hazardous materials

I reviewed 8 children files and found 3 files missing immunization records, 3 files missing health statements and 1 file missing the food allergy emergency plan.
