The operation has been delinquent in payment of the background check fee since 3/2/25.

During the walk through the director was not present and caregivers did not know who was in charge. This was corrected at inspection when the director showed up and let them know who is in charge in her absence.

The child's swelling progressively increased throughout the day and the parent was not notified.

1 child was missing immunizations in their records that are required to be on file.

Licencing observed a caregiver drop off 2 children and the child safety alarm was not working.

During naptime 3 children were observed to be sleeping on other students assigned cots.

Procedures for supporting inclusive services to children with special care needs was not in operational policy.

There were 6 infants between 12 months -19 months observed with one caregiver. The classroom was out of ratio by 1 child. Note: The oldest child was moved into another classroom.

A caregiver was observed walking out of the classroom leaving the children unsupervised to answer the phone.

The sprinkler system was last inspected on 05/26/22.

A caregiver was left in charge of two seperate classrooms and she was observed sitting between both classrooms during naptime.

A child was observed sitting in a walker. I informed caregivers that walkers are prohibitied items and caregivers removed it from the classroom.

During inspector walkthrough a caregiver was observed washing infant bottles in the handwashing sink.

Two out of four employee records reviewed were lacking a signed and notorized affidavit of employement.

A caregiver did not sanitize the diaper changing station between diaper changes.

A caregiver did not wash their hands after changing a child's diaper.

The caregiver did not wash a child's hands after they were diaper changed.
