A caregiver without first aid training was observed having unsupervised access to children during naptime.
It was observed that the carbon monoxide detector was missing a battery and was not working properly. Note: A new carbon monoxide was purchased, installed, and tested in the center during inspection.
An employee was supervising children at the center with a background check pending eligibility determination. Note: Employee was immediatly removed from the center and inform to not return until notification from the CBCU is recieved.
The fire inspection had expired, the last inspection was conducted on March of 2022. NOTE: This was corrected at inspection when the fire department was called and they conducted the fire inspection right then and there.
The center has cots available. However there were children sleeping/laying directly on the floor.
Based on information gained during the investigation a caregiver was playing games on her cell phone.
