A child was left unsupervised in a restroom in the pre-k room when the caregiver left to the outdoor playground with the rest of the children.

Single-use cups were being reused through out the day to provide children drinking water in the afterschool room. The cups were labeled with each child's name. Note: This was CORRECTED during the inspection as the single-use cups were disposed.

Spider webs were observed in the corner of a wooden fence in the toddler playground area. This posed a hazard as the spider webs were within children's reach. Note: This was CORRECTED during the inspection as the spiderwebs were removed.

The first aid kits in the transportation vehicles were incomplete. The first aid kits in 2 of 3 Vans were missing cottons balls and the first aid kit in1 of 3 vans was missing a thermometer. Note: This was CORRECTED during the inspection when the missing items were obtained and placed in the first aid kits.

The most recent child care regulation inspection or investigation form was not posted. Note: This was CORRECTED during the inspection as the most recent inspection form was posted.

The director was missing annual training in transportation safety. The training was last complted on August 2021.

1 of 6 children's immunization records reviewed was not current.

The emergency medical transport and treatment authorization forms were missing for 5 children being transported. Note: This was CORRECTED during the inspection as the required forms were obtained and placed in the transportation vehicle binder.
