The bathroom in the two year old room had 2 used pull-ups on the floor. This was CORRECTED AT INSPECTION when the diapers were disposed of.
The operational policies were lacking information on applying and providing sunscreen and insect repellant. This was CORRECTED AT INSPECTION when the director added the information to the operational policies.

During the course of the investigation, it was determined that the operation stained a playground fence while children were playing on the playground.
A caregiver left children unsupervised when they exited the classroom to answer the front door.
Two children's admission forms were missing an acknowledgment on the section of the health statement for the status of the child's physician examine.
An outdoor sink was found to have standing water.
A wooden fence on the back of the play yard was leaning and had broken boards.
Two bicycles on the merry go round were missing the pedals. A push car was missing the cover to the battery compartment, and a water table had a broken plastic.
Several electrical outlet plugs at the operation were without safety covers.
The refrigerator and sink had dry marks, debrie and were dirty. The floor was cluttered with items; such as milk crates, highchair, bag of rice, cans of food and cleaning solutions creating a hazard.
Cleaning supplies were observed next to canned foods on the kitchen floor.
The fire inspection was not available for review because the operation did not pass inspection.
There were five infants and one toddler in the infant classroom with one caregiver.
A child was observed leaving the Pre-K classroom, unlocking the front door and opening the door to a caregiver.
The access door to the attic was detached from the ceiling in the infant classroom. The hinge on the door to the infant room was loose.
There was s swiffer wet jet mop in the infant room with a cleaning solution bottle by the wall close to the door.
The caregiver in the toddler room was observed leaving children unsupervised in the classroom while attending the front door and when taking children to the restroom. The infant caregiver was also observed exiting the infant classroom to talk to the toddler caregiver.
A slide on the playground did not have 6 inches of loose-fill. The loose-fill at the exit of the slide measured at 1 inch.
One infant in care did not have an infant feeding schedule available for review. Another infant's feeding schedule had not been updated since March.
2 slides on the playground did not have a 6 ft use zone on one side of each slide.
A caregiver in the infant room did not wash the child's hands after a diaper change. This was CORRECTED AT INSPECTION when the infant was taken to the sink to have their hands washed.
Re-cite, play equipment did not have the required loose-fill .
There was a tray with knives, pizza cutter and other kitchen utensils that were at hands reach of children in the kitchen. The kitchen is across the children's restroom and the kitchen door was open.
A caregiver present and in the Pre-k classroom had a pending background check. Caregiver's time sheet showed a clock in for date 6/2/2023, 6/5/2023, 6/6/2023, 6/7/2023, 6/8/2023, and 6/12/2023. Note. Caregiver was ask to leave on 6/12/2023 and did not .
There were two play structures with slides that did not have loose-fill.
