Infant bottles were either not labeled while others looked like the label had been washed off as you could see partial letters on the bottle. I spoke with the operation about labeling bottles.

Observed four cribs in the infant room, All four cribs were being used by other infants while there were two infants who were asleep and did not have a crib to be placed in. The operation stated that some of the older infants usually sleep on mats on the floor, but they had placed them in the crib today.

The operation has a caregiver who is under the age of 18 who has graduated from Highschool but is not enrolled into a childcare career related program.

Observed a toddler taken out of a crib and another infant laid down in the same crib with the same crib sheet. This was corrected when it was discussed with the caregivers that the crib must be sanitized and the sheet changed prior to use by another child.

Observed two infants sleeping, one in a swing and one in a bouncer seat. This was corrected when the infants were removed from the restrictive devices.

Out of four staff files reviewed two did not have in person training in CPR as required..

The operation has failed to update the Background Check Person List as the submission for a subject has been pending since 9/22/2021. The list was last validated on 9/21/2021. **This was corrected at inspection when the subject was removed from the list.
