Based on the information gathered during the investigation, it has been determined that children were left unattended, and caregiver was not at a physical proximity which did not allow visual and/or auditory awareness of the children in care. Caregiver left the classroom where children were in care and children were walking in and out of the classroom.
Based on the gatherings during the investigation, it was determined the operation failed to notify the licensing immediately but no later than two days after a child was hurt and required medical treatment by a health care professional.
The nap cots were not labeled during the visit.
The operational policies were missing several items to inlcude the promotion of indoor and outdoor activities.
The emergency prepardness plan did not have a name and address of an alternate location away from the operation.
