Based on the information obtained during the investigation. It was determined that a child was left unsupervised in the classroom between 2-4 minutes.

The storage chest on all the playgrounds did not have a id support on them and it was open and accessible to the children. Note: This was corrected when the director put a safety lock on each storage chest that was on the outdoor playground. 10/12/2022.

The light fixture was hanging on one side in the in the preschool 2 (3-4yrs.) classroom. Note: This was corrected at inspection when the handy man came and fixed the hanging light fixture. 10/12/2022

One out of five files did not have a CPR/First Aid card in the file.

Two out of 10 file that were infants, did not have the safe sleep form.

Based on preponderance of evidence obtained during the investigation, there were sufficient evidence that the caregiver did not use good judgment when she picked two children up by only one of their arms and with only one of her hands.

A classroom was observed to be out of ratio.

Ten children's files were reviewed. One file did not have the complete emergency contact information documented. One file did not have the emergency authorization form completed. One file did not have the health provider statement. Two of the files did not have admission dates documented. One of the files did not have health care information documented.

Ten children's files were reviewed. Two of the files have not been updated in a year.
