Based on the information gathered through interviews conducted by the staff, parents, and documents received. It was determined that a child was found unsupervised in a classroom by a parent.
A caregiver was observed in the classroom without an active background check. Note: This was corrected when the caregiver left the operation and their background was resubmitted.
A two-year-old child was left outside the classroom unsupervised.
A 4yr old child was left in the classroom unsupervised.
A 3-year-old child ran out of the classroom into the hallway while the teacher ran after the child leaving the classroom unattended. A staff close by heard and saw what was happening and made the decision to go inside the classroom with the children.
A child affidavit was observed to be expired.
The 12-18 classroom had a toy bin with outside debris in it. The 2-year-old room had a cracked toy bin with toys in it. These were corrected during the inspection.
Three infants under the age of 12 months were observed to have a type of attachment hook on their pacifier such as a beaded string and a small stuff animal while still in the crib. This was corrected at inspection when the director detached each one from the pacifier.
A 5-year-old child was using the restroom in the gym while no teachers were present in the gym to provide proper supervision.
A staff did not conduct a name to face while lining the children up to transition to another room to ensure all children were accounted for. This resulted in a child being left in a classroom alone for approximately one to two minutes.
Several medication records did not have the first and last name of the person administering the medication.
There were several rooms observed where the cots were not labeled. This was corrected when the caregivers labeled each cot that was missing a child's name.
Medication wasn't returned when a child withdrew from the center. This was corrected when an email was sent to the parent to come pick up the medication.
The equipment for cribs was not available for review.
