The operation did not write the incident report on September 15 around 5:00pm for alleged incident that occur inside the pre-k classroom that involved inappropriate discipline against a child.

The parent did not sign and receive a copy of the incident report within fourty-eight hours acknowledging they were incident report in a timely manner.

Infant feeding instructions were not updated at the 30 day mark.The infant feeding instruction sheet was dated 10/1/2025.

Two infant feeding instruction sheets did not have a date. One infant feeding instruction sheet did not have a parents signature.

A review of the Employees Who Left Employment in 2024 data revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

Ten personnel records were reviewed and missing the following requirements: Affidavit of Employment Proof of Education

The first aid/CPR training is invalid for 3 caregivers.

Health inspection was last completed on 4/19/2023 and is now expired.

Fire inpsection was last completed on 3/28/2023 and is now expired.

Infant feeding instruction for 5 infants enrolled in Infant class do not have updated feeding instructions.

One caregiver providing care to children during the inspection did not have an eligible background check.

A food allergy emergency plan was not signed by the parent or physician for a 4 year old child. Note: Center contacted parent immediately.

3 out of 10 children's files do not have the emergency contact's address provided.
