During the inspection, evidence was obtained that a child was left unattended in the bathroom alone while the class was outside.

The operation last fire inspection was on 2/8/2024.

During the inspection the director stated the child was left unattended in the classroom. The child was using the restroom when the class left the room. The child walked out the classroom alone.

The victim child and caregiver admitted to the child being left outside unsupervised.

The annual sanitation inspection was last conducted on 04/30/22. It was due for inspection by 04/30/23.

Based on the findings of the investigation, a caregiver forced children to sleep during naptime.

Based on the findings of the investigation, a caregiver used harsh language when speaking to the children, especially during naptime.

The operation did not complete their annual fire inspection since 2021.

Four out of the ten operational staff have not complete their annual twenty-four training hours required by childcare licensing.

The operational staff demonstrate poor judgment by returning a coin to a four year old child which led him swallowing it and require hospitalization the following day.

One operational staff does not have hand on skill training for pediatric CPR.
