Based on information gathered during the investigation, it was determined that a caregiver kept magnets from a broken toy, which later was ingested by a child while in care.
The operation has one child with a food allergy listed but does not have a food allergy plan on file at the operation.
Five caregivers have online only CPR on file at the operation.
The operation does not have documentation of a current gas leak inspection.
A child was left unsupervised for approximately 5 minutes. The caregiver did not count them before the transition to the next room.
The caregiver fed a bottle to an infant that was intended for a different child in care.
