Wooden chips were below proper depth around the school age playstructure area.
One child's admission form did not have a parent's signature.
A feeding instruction form was incomplete and did not have the correct date. In addition, March feeding instructions were not completed.
A daily report did not detail the amount of food not consumed by a child.
A caregiver was using a cell phone while a child was in care.
At the time of the inspection the playground structure had a crack in a side panel that was small, but had a rough edge and a second panel was secured with bolts that extended 8 threads out from the structure. The bolts were on the exterior portion of the panel and not easily accessed by children, but would be reachable by children's hands. On the playground area there was a loose pile of 2 garden hoses.
At the time of the inspection a play equipment structure modeled after a fire truck had warped siding that created a gap where two pieces of the siding met. This gap was small enough for children's fingers to be caught in.
At the time of the inspection no infants present had current written feeding instructions signed by their parents.
At the time of the inspection an infant under the age of 12 months was observed with a pacifier clip line attached to their clothing while they slept in the crib. The clip was removed immediately by the caregiver.
Based on information obtained through investigation, staff were aware of an incident occurring and did not take steps to report the occurrence to Childcare Regulation. The operation did take steps to address the concern and add preventative measures.
At the time of the inspection the operation had not had a fire inspection within the last 12 months as required.
At the time of the visit 2 children were observed laying on their nap cots playing on their personal tablets.
At the time of the visit 6 of the 10 children's records reviewed did not have the required information for emergency contacts.
At the time of the inspection 5 of the 10 records reviewed had missing immunization records or immunization that was not up to date.
At the time of the visit it was found through observation of dish soap and a bottle scrubber, and discussion with caregivers about their practices, that the infant handwashing sink was being used to also wash infant dishes such as bottles and spoons.
At the time of the inspection 2 children with noted food allergies in their file did not have the required doctor note with instructions on what to do if a reaction occurs.
At the time of the inspection it was found that no infants in care had written feeding instructions from the last 30 days.
Based on information obtained through investigation it was found a caregiver had been present in a classroom without a completed background check and without the operation having been notified of the background check results.
At the time of the inspection the loose fill on the toddler playground measured less than the required 6 inches at the base of the slides of the structure.
Based on information obtained through investigations 3 children reported that a caregiver squeezed their hand or shoulder when guided to a time out in a way that hurt them.
Based on the information obtained a caregiver did not relate to children respectfully by raising her voice and talking in the child's face while re-directing behavior.
Based on information provided by DFPS a caregiver was using their personal cell phone during an incident where a second caregiver was inappropriately disciplining another child. The second caregiver did not maintain adequate supervision of the class while on their cell phone.
Based on information provided by DFPS it as found that 2 caregivers were found reason to believe for Neglectful Supervision and Physical Abuse.
Based on information provided by DFPS it was found that the director did not supervise staff appropriately when 2 caregivers were found Reason To Believe for Neglectful Supervision and Physical Abuse.
Based on information provided by DFPS a caregiver stated that for discipline blankets are taken away from children at nap time because they are seen as a privilege.
Based on information provided by DFPS and video reviewed a caregiver was observed yanking a child in the classroom and holding the child's wrists tightly, to the point of leaving a bruise.
Based on information provided by DFPS a caregiver was observed and said to have raised their voice to children in care to get them to behave.
Based on information provided by DFPS 1 caregiver did not use self control when they pushed another caregiver to chase a child who was running away from them. A second caregiver did not use good judgement when they witnessed a caregiver using excessive force in discipline and did not intervene or notify the operation administration.
