11 of the 35 child safety seats were expired. No children were transported and the operation corrected the seats when they removed all the expired seats from the vehicles.
In 3 of the classroom, peeling paint and wall depressions were observed. This was corrected during the inspection, when the staffed painted the areas and provided coverings until the financial burden could be met.
The carpets, trash cans, 3 bathrooms needed to be cleaned and sanitized. There were stains on the carpets in the classrooms, and smudges on the floors and walls. The trash can in one of the classrooms needed to be wiped down and the presence of urine was in three of the bathrooms.
Two toilets in the classrooms were observed with feces. There were no paper towels in one of the restrooms. This was corrected at the inspection when the caregiver flushed the toilet and put paper towels in the restroom area.
One of the children records did not contain a health statement.
During the investigation inspection there were missing ceiling tiles observed in the toddler restroom area and rust build up in some of the toilets.
The infant room contained a 7 week old and 21 month old in the same classroom. This was corrected at the inspection when the 21 month old was moved to a different classroom.
During the monitoring inspection it was found that the director's CPR and first aid training expired on 10/31/2023.
The transportation vans did not contain an electronic child safety alarm.
During a walk-through of the childcare facility, an electrical outlet was observed to be accessible to infants in the infant classroom. The classroom was observed to have walking infants. A safety cover was immediately placed over the electrical outlet, bringing the facility into compliance.
After reviewing infant feeding instructions, three infants not yet ready for table food did not have updated feeding instructions on file for review. The most recent feeding instructions were dated October of 2022.
During a walk-through of the childcare facility, an infant was observed to be asleep and swaddled. The infant was immediately unswaddled when addressed with caregivers. No sleep exception form was on file for review regarding swaddling.
During a walk-through of the childcare facility, a hallway needed to be cleaned as there were infant gates, blankets and debris on the floor which presented a health hazard for children in care. This was corrected at inspection when the hallway was cleaned.
