A review of the Employees Who Left Employment in 2024 Survey data from January 17, 2025, revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.
Based on the evidence obtained during the investigation, caregivers were observed grabbing a child by the arms.
I observed an 18 month child sleep with a bottle in their mouth. A photo was taken and the bottle was removed.
I observed a water hose on the play ground. This was corrected when the water hose was removed.
A caregiver failed to proper communication with another caregiver before leaving playground.
