On 7/23/2026, video footage showed infants being removed from a table at 11:40 AM. Two infants were left in the bucket chairs from 11:40 AM-11:54 AM, seconds from 11:55 AM (15 minutes). The teachers were observed putting the other three children to sleep while the other two children sat there for nearly fifteen minutes until one teacher went to get them. The teachers did not bring all the children down during the same time frame. The children were observed still buckled in their seats.

On 7/23/2026 video footage from 7/23/26 was viewed and a teacher was observed feeding children at a table with one child in the back of the room playing. The teacher was observed to have her back to the child and did not turn around for 5 minutes. The teacher then walked to the sink behind the table and the child who was playing in the back ran to the table and attempted to take another child's food. That child pushed the other child away, causing them to flal to the floor. The angle of the camera did not catch the child on video, but it was unknown if the cihld was crying. The child was observed to have gotten up less than two minutes later, and the teacher's back was still to the children. On 8/1/2026 the director had a meeting with the staff where the issue of supervision was addressed. The caregiver did attend this meeting as per the sign in log. This was corrected before today's inspection. On 7/6/2026 a caregiver left children unattended for two minutes in a classroom while she went to the front to pick up a child.
