An incident report was not completed at the time of the incident.
The operation did not inform a parent immediately when a child was left in the classroom for an undetermined amount of time.
The operation did not notify Child Care Regulation when a child was unsupervised for an unknown amount of time in the classroom. The camera was reviewed by the director when the incident occurred, and the child was seen playing in the classroom under no distress.
Two staff did not conduct a name to face while lining the children up to go outside while in the room before opening the door and when outside on the porch area to ensure all children were accounted for before going out on the playground. This resulted in a child being left inside the classroom unsupervised for an undetermined amount of time.
The 12-15 month room was observed serving food on a bare group table setting with no individual plates/bowls.
A six month old was observed sleeping in a swing which is a restrictive device upon entering the room. This was corrected when the director moved the infant out of the device.
During the course of the investigation, it was determined that all bottles and cups were properly labeled. However, a caregiver failed to ensure that the correct bottle was given to the right child, resulting in an infant being fed another child's mother's breastmilk. Note: This caregiver is no longer employed.
Operation staff did not do a name to face count or a sweep of the playground when transitioning from outdoor play to indoors, resulting in a child being left unattended on the playground for approximately 9 minutes. The operation terminated the caregivers responsible for the incident.
A few bottles were missing the first name last initial. This was corrected when the bottles were labeled.
At the time of inspection, the caregivers in the infant room did not know how many children were in their care. She stated there were 7 in care when there was 8.
A pair of scissors was found on the diaper changing table accessible to children. This was corrected when the scissor was put away.
