A Review of the Employees Who Left Employment in 2024 data from January 22, 2025, revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.
A 9-month-old was in a sleeper rocker without safety straps. Also, a 12-month-old was sitting in a bucket chair at an infant feeding table without the safety straps fastened. The child was removed from the sleeper rocker at inspection and the safety straps in the bucket chair were fastened at inspection.
Two infants that are both 5-months-old were wearing pacifier clips. One of the pacifier clips was made of beads. The pacifier clips were removed at inspection.
An infant and toddler classroom both had soiled diapers in the trash can without a lid. This was corrected at inspection.
Chemicals were being stored in a cabinet under the sink in the toddler room. The cabinet did not have a lock and the chemicals were accessible to children. This was corrected inspection.
A caregiver did not report the cause of a child's injury, although she witnessed the injury. The caregiver supervising the child did not report the incident of the shelf falling on the child to the director.
