During the inspection I observed a 15-month-old- combined with children as old as 3 years old. This was corrected when an additional caregiver arrived to take the older children.
The operation failed to report an allegation of a child being placed at risk to licensing within 48 hours as required.
A child with peanut allergies does not have an emergency plan.
The director failed to complete all required training within the annual year.
Five staff files were reviewed, and Five caregivers are missing the required special topic training.
In the toddler's classroom, there was a fan on the floor that was accessible to the children. Correction: A caregiver removed the fan and stored it away from the children.
The playground for school-age children has a barbecue pit, and a bottle of lighter fluid placed on top, making it accessible to the children. Correction: A caregiver removed the barbecue pit and lighter fluid, placing it out of the children's reach.
CCR observed three school-age children entering the building through the back door to the playground to get a drink of water without supervision. CCR also observed a child left alone, sleeping in a classroom with the door closed.
Near the school-age basketball court, there is a bucket, trash can, two black trash bags, five bricks, broken wood, broken foam cups, and a pipe. There is a hole in the ground near the children's playground area collecting water. In the Pre-K classroom, two large boards leaning against the wall and are accessible to the children.
In the toddler's classroom, there was a fan on the floor that was accessible to the children. Correction: A caregiver removed the fan and stored it away from the children.
In the toddlers classroom, there was a mop bucket filled with water and not being used. Correction: A caregiver removed the mop bucket and stored it away from the children.
Garbage trash without a lid was accessible to children.
The children playground loose fill is less than an inch ruler.
A water hose was observed on the playground and not in use.
It was revealed that the caregivers were yelling at the children at the operation.
During the inspection I observed a caregiver without a cleared eligible background check supervising 4 infants. This was corrected when the director removed the caregiver from the classroom.
During the walkthrough of the playground I observed a bbq pit, waterhose and debris next to the outdoor shed as well as a broken fence board. The bbq pit was removed during the inspection and the fence board was repaired.
Through statements received throughout the investigation determined that a caregiver used profane language with a relative who was a child in care.
