I evaluated 4 children's enrollment records. I observed the following: 1. Three children's records did not have contact information for the child's physician or nearest hospital. 2. One child's file had emergency contact information listed, but did not have the person's name listed.
I did not observe enrollment files for two children observed at the operation during the inspection.
Child Care Regulation was not notified of the new site director for the operation.
