The vehicle was returned to another operation location, where the child was left on the bus for approximately 15 minutes. Staff were unaware of the child's absence from the group until the child's parent came to pick them up from care.

The caregivers did not follow the operation's established protocol for accounting for children following a field trip. They failed to do a count of children, did not conduct a name to face count, and failed to conduct a full walkthrough of the bus which resulted in a child being left on a bus alone for approximately 15 minutes.
