A sampling of children records were evaluated. Two children's records lacked the complete address (city and state) of their designated contact. Eight children's records lacked authorization to obtain emergency medical care and to transport the child for emergency medical treatment. Eight children lacked documentation that their parents received and reviewed a copy of the operations, operational policies.

The operation has an Epi pen on file that was not in the original container labeled with the child s full name and the date brought to the operation.
