101229.1 Sign In and Sign Out (b)The person who brings the child to, and removes the child from, the center shall sign the child in/out. This requirement is not met evidenced by: Directed stated she sent out an email to all parents reminding them the imprtance of signing in and sign out the child/children. Plan of correction due by 11/16/2023 to LPA Dean at Dean.thompson@dss.ca.gov or the regional office. Based on sign in/ sign out sheets reviewed on 11/7/2023 and 11/9/2023 showing 36 children present but only 33 children were signed in poses/posed a potential health, safety or personal rights risk to persons in care 8
Based on LPA's observation, interview, and record review, the licensee did not comply with the section cited above by means of not having parent consent forms completed, and physicians' information labels on medication for children in care which poses an immediate health and safety risk to persons in care.

Based on interviews from all staff, it was disclosed that they did not observe when or how the injury occur. They believe it happened when the child throws themselves on the bathroom floor. This is a potential risk to the health and safety of children in care.
