Based on observations, the licensee did not comply with the section cited above in 1 out of 3 medications were expired which poses a potential health, safety or personal rights risk to persons in care.
Licensee has corrected the affected fixture by removing it on 12/08/2023. Licensee will provide the proper documentation for the Action Lead Exceedance directives to the Department by 12/02/2024. Licensee failed to notify the department of the lead exceedance results within two weeks of the report received.
Based on observation and interview, Licensee did not comply to this section cited above. LPA observed that Licensee has made construction alterations in the facility without notifying the Department which poses a potential health, safety or personal rights risk to persons in care.
