This requirement was not met as evidence by: 1 Licensee will have all parents sign a sheet stating they received the Type A citation and that sign-in sheet will be sent to the department by the plan of correction date. (3) The licensee shall require each recipient of the licensing report described in paragraph (1) pertaining to a complaint investigation to sign a statement indicating that he or she has received the document and the date it was received. (4) The licensee shall keep verification of receipt in each child's file. Licensee will have all parents sign the LIC 9224 and submit proof to the department by the proof of correction date. Based on observation and record review the licensee did not comply with the section cited above by not having all parents sign the LIC 9224 when the Type A citation happened (OCT 2023) and when they have newly enrolled students into this facility which poses a potential health, safety or personal rights risk to persons in care. 8

a staff caused injury to an infant child while trying to get the infant child to sleep during nap time on July 17, 2023. The Facility terminated the staff person as a result of the incident. This presents an immediate risk to the health, safety, or personal rights of children in care. 8 Licensee must provide copies of this report to parents/guardians of children in care at this facility and to parents/guardians of children newly enrolled at this facility during the next 12 months per the AB633 requirements.
