Responsibility for Providing Care and Supervision. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidenced 1 Staff 2 and Staff 3 are no longer employeed at the facility. Facility has implemented a requirement for staff to do a double roll call in the morning, and for staff to stay an additional 15-20 minutes after children have been dismissed to ensure that no child has been left alone. by an Unusual Incident Report received on 10/28/2025. Facility self-reported that on 10/27/2025, Child 1 was left alone in the room from approximately 7:40am-9:3ham. This posed an immediate risk to the health, safety, and personal rights of children in care. 8

The licensee shall provide care and supervision as necessary to meet the children's needs. No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. Site Coordinator stated they have implemented having a staff at the children's bathroom and double checking the count of children going to restroom by way of walkie talkie and returning from restroom. LPA observed and witnessed the new procedure being implemented at time of inspection. This requirement was not met as a child was left unsupervised in the bathroom for approximately 5 minutes. This is an immediate risk to a child. 8 Deficiency cleared at time of inspection. Site supervisor was given the LIC9224 and explained at time of inspection.
