Based on interviews the facility did not ensure the supervision of a child during restroom use. Door to the restroom was closed prior to ensuring all children had exited, which posed a potential Safety rights to persons in care.
Based on interviews, record review and video footage, on 3/23/23 at approximately 2:50 p.m. a staff member (S1) slightly pulled a child (C1) by the hand at a pace C1 had difficulty keeping up with. The poses a potential health, safety or personal rights risk to children in care. 8 Lead Teacher Diana Padilla states that she will have the director conduct a staff meeting on childen's personal rights and she will send LPA a copy of the meeting agenda and staff sign in she via email by 5/5/23.
