... Assistant director provided documentation of 10/25/24 staff meeting related to the allegation that addressed training on supervision, prevention, and health and safety review. This deficiency is cleared. Based upon staff interviews and video evidence, C1 incurred bite marks during an incident with another student. This injury went unnoticed and unassessed by the staff, which posed a potential health, safety and personal rights risk to children in care.

Based on staff interviews, a child was refused entry when parent and child arrived a few minutes late. Admission Policies did not include exclusion of child due to late arrival. 8 Site director shall submit a plan of correction no later than 3/29/2024. Request Denied

Based on staff interviews parents were not allowed inside the classroom due to the COVID pandemic. 8 Site director shall submit a plan of correction to the department no later than 3/29/2024.

Based on record review, the licensee did not comply with the directive above, as one water outlet (corner hand wasking sink outlet in room 3) out of ten outlets tested with an Action Level Exceedance (ALE). This poses a potential Health and Safety risk to children in care. 8 Facility is scheduled to have the faucet retest on 11/21/2022. Facility shall submit the test result along with the LIC9276 to LPA no later than 11/30/2022.

Staff did not prevent daycare child from biting another child.

1. Licensee does not allow parents to enter and inspect the day care.

2. Admission policies - staff refused entry to a child when he arrived 3 minutes late.
