The Center Director would go into the classroom every so often to assist children going to the bathroom but was assisting other classrooms in the center while S1 was left alone with children. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Based on record review, six children's records were reviewed three children C2, C3, C5 were missing one or more doeses of certain immunizations. C4 was missing Blue CDPH 286.
