This requirement is not met as evidence by: By Plan of Correction due date of 10/09/2025, Director will provide copy of training materials and meeting minutes regarding supervision. Based on records review and interviews, Child 1 was left unsupervised and wandered intto the parking lot area which posed an immediate health, safety, or personal rights risk to person in care. 8
