Responsibility for Providing Care and Supervision: The licensee shall provide care and supervision as necessary to meet the children's needs. No child(ren) shall be left without the supervision of a teacher at any time. Supervision shall include visual observation. This requirement is not as 1 Interim supervisor stated the facility will send LPA a written statement with the facility procedure for ensuring children are supervised at all times, especially during transitions. Also, facility will conduct a mandatory training by 06/19/2026, with all staff regarding supervision. evidenced by: Based on video footage, interviews conducted and records reviewed, the licensee did not comply with the section cited above as a child was left unsupervised outside the classroom which poses/posed an immediate health, safety or personal rights risk to persons in care. 8 Interim supervisor will submit the meeting agenda to LPA, signed and dated by all staff. Interim supervisor states will also conduct unannounced classroom observations to monitor compliance.

This poses an immediate health and safety risk to children in care. 8 Care and Supervision and will have training with a licensing staff member. Per interviews and documents received, it was confirmed that there were 3 CDT (child development technicians). 1 Director states will submit to the department a new template of a name to face with more detail. For example the staff will document the child name, time, date and identified activity. The child development supervisor will meet with all staff to review template and licensing regulation regarding left alone in a classroom with 14 children and no qualified teacher on 12/18/2026. 8 Care and Supervision and will have training with a licensing staff member. Will also review training video from licensing website. Director will submit the plan and time line of these corrections by 03/16/2026.
