Based on observation, interview, and record review, the licensee did not comply with the section cited above in 2 out 3 staff did not complete the AB1207 Mandated Reporter Training and 1 out 3 did not have the training completed which poses a potential health, safety or personal rights risk to persons in care.

101700.3 (b) Testing results with fractional ppb readings of 0.5 ppb or greater shall be rounded up to the nearest whole number, before comparing to the Action Level. (1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. During visit, facility had to re-post their lead results and make outlet B inacessible to children in care. Lead documents will be submitted to LPA via email at dianna.valdezsantana@dss.ca.gov. Parent notification was sent via app "Learning Genie" on 1/9/23. This requirement is not met evidenced by record review of the lab report analysis from Outlet B taken on 12/20/2022. Results indicated levels of lead in exceedance. This poses a potential risk to the health and safety of children in care. 8 Administration will retest, replace or repair outlet B and will submit resampling results to LPA.
