The licensee did not met the regulation: 1 The director will hold a training with staff about communication with children and ways to help children manage challenging behaviors. The director will provide materials and a sign in sheet showing staff participation. Materials will be sent by due date. Based on interviews conducted, the licensee did not meet the regulation above, which poses an immediate risk to the health, safety, and personal rights of children. 101700.3 (b) Testing results with fractional ppb readings of 0.5 ppb or greater shall be rounded up to the nearest whole number, before comparing to the Action Level. (1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. During visit, LPA confirmed lead results were posted, and lead documents were submitted. Director will remove, replace or repair outlet E and will submit plan of correction by 02/13/2023. Please refer to the 101704 Lead Action Level Exceedance Response Requirements PIN provided today. This requirement is not met evidenced by record review of the lab report analysis from Outlet E Rm2(b) taken on 12/03/2022. Results indicated levels of lead in exceedance at 5.6 This poses a potential risk to the health and safety of children in care. 8
