During a visit to the facility on 07/23/2025, LPA observed 3 cracks and pieces missing from a bench in the outdoor play area. LPA sat at the table during 1 Facilty representative stated they will throw out the broken bench, and submit a picture via email or text message showing the area is now free of hazards. children's outdoor playtime today and saw the same exact cracks still there. Children were seen playing around the cracked bench. This was addressed under a Technical Violation (TV), but is now being cited as a Type A citation. This poses an immediate health and safety risk to the children in care.
