Interviews and observations where staff did not supevise children to prevent injuries and children are not being supervised while going to the bathrooom, which causes and immediate health and safety risk to children in care. 8 offer alternative shorter scheduled activities; eliminate cell phone distractions; new bathroom policy will be submitted. Director will have staff meeting on Friday, March 14, and will submit new supervision and bahtroom policy by March 18, 2025.
Interviews revealed that Staff did not verbally or in writing notify parent of a bite on C1's forhead. Facility will conduct a personal rights training to all staff, which will also include the CCLD video on Personal Rights and appropriate communication around children. Facility will send LPA staff signatures showing their attendance and understanding of the meeting. Interviews revealed that Staff 1 spoke negatively and inappropriately about children in care while children could still hear Staff 1 on multiple occasions. This poses an immediate health, safety, or personal rights risk to children in care.

LPA learned through record review that a child was left in the play yard without staff supervision on 2/2/24. This poses an immediate risk to the health and safety of children in care. 8 send LPA the signatures from all staff acknowleging that they completed watching the video. Civil Penalty of two hundred fifty dollars($250.00) will be assessed on the next page (LIC 421FC).
LPA learned that a child was left in the play yard without staff supervision 2/2/24 & was not reported to CCLD. This poses an potential health & safety risk to children in care.
Licensing was notified of an incident from facility staff and by unusual incident report that on 8/23/23, C1 was found alone in the facility parking lot for approximately 3 minutes. This poses an immediate risk to the health and safety of children in care. 8 consist of supervision practices. Director will send LPA signatures of all staff members who will be attending the staff meeting. Civil Penalty of five hunded dollars($500.00) will be assess on the next page (LIC 421M).
Interviews revealed several times that children were yelled at when a staff member was trying to correct their behavior. This is a health and safety risk to children in care.
Interviews revealed that staff held a child down to a chair and staff grabbed a child by the arm to hold down on a nap mat. This was a health and safety risk to the children in care.
D1 hired 2 staff members for floating in the afternoons based on ratio. D1 will email LPA a staff schedule and staff unit log by closing on 06/08/2023. Interviews revealed that the facilty was operating out of ratio in the afternoons. More than 12 children were under the care of 1 teacher. Aides were supervising children in the capacity of a qualified teacher. This posed a health and safety risk to children in care.
The front door was repaired before the date of findings. On 04/20/2023, LPA observed a new door sign, an operational pad for door codes and a repaired front door. Additionally, D1 installed new door locks and knobs for the classrooms to ensure safety. A broken front door that LPA observed on 04/17/2023. Children attempted to exit and adults were able to enter without permission. This is a potential health and safety risk to children in care.
Based on LPA observing incomplete staff files and therfore, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.
Based on LPA's observation and record review, 3 adult staff members were not associated to the license and 1 adult staff member was not background cleared nor associatedf to the license which poses an immediate health, safety and or personal rights risk to chidlren in care.
