LPA observed Child 1 climbing a book case and eating the tip off a marker without teacher's noticing. It was revealed that Child 1 was eating sand from the sandbox when under staff supervision. This poses a potential health, safety and personal rights risk to children in care.

Director Moore states that a staff training will be done regarding diapering and will work with staff to create a more throrough plan to organize potty training to avoid diaper rash and incorrect diapers being used. Director Moore will send LPA signatures of all staff in attendance, showing they received... Child 1 was receiving diaper rashes while in care as well as wearing incorrect diapers. Parents share same concern of their child receiving diaper rashes and diapers that do not belong to the child. This poses a potential health, safety and personal rights risk to children in care. 8 ...the training.

Director stated they will request a waiver to allow for commingling for the first and last 30 minutes of the day for preschool and infants while maintaining the ratio for the youngest age served. Through interviews and document gathering, LPA learned school age children and preschool children comingle in the morning and evening. This poses a potential health and safety risk to children in care.

This requirement has not been met by evidence: Facility self-reported the incident to the RO. This is considered 1 Director has conducted trainings with staff to prevent any similar incident from occuring in the future. LPA cleared the deficiency. as an immediate risk to the children in care. 8

101229.1 Sign In and Sign Out (b) The person who brings the child to, and removes the child from, the center shall sign the child in/out. This requirement was not met as evidence by: Director will create and provide a new policy to parents informing them that they need to personally sign their children in/out of the center and that failing to do so will require them to return and possibly incur a fee. Staff will receive disciplinary action for signing children in/out. Interviews conducted by LPA consistently referenced on-going issues with parents not signing their children in/out of care, and that staff were then signing the children in/out of care when parents failed to do so. This poses a potential risk to the health and safety of children. 8 Director will provide copies of the new policies to LPA.

The facility has turned off the water line to the outlet with the exceedances and placed a notice indicating it is not allowed for use. The facility plans to replace the drinking fountain and have it retested. Proof of a new drinking fountain or a test indicating levels of 5 ppb or less will be Lead test results received by Licensing on 09/9/2022, tested on 06/22/2022, showed an outlet in the facility had lead levels exceeding 5 parts per billion (ppb). This poses a potential risk to the health and safety of children in care. 8 sent to LPA by 10/14/2022.
