Based on observation, interview, record review, the licensee did not comply with the section cited above in 3 out of 6 staff did not have criminal record clearance, which poses an immediate health, safety or personal rights risk to persons in care.

Based on observation, interview, record review, the licensee did not comply with the section cited above as in the facility did not have a functioning carbon monoxide detector, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, interview and record review, the licensee did not comply with the section cited above as in the facility did not have License, Notification of Parents' Rights (PUB 393), Personal Rights (LIC 613A), PUB 269, Daily Schedule, and snack menus posted in at the facility which poses/posed a potential health, safety or personal rights risk to persons in care.
