Based on documentation, video footage and interviews, the facility staff did not comply with the section cited above, as a child was left unattended in the classroom for approximately 2 minutes before the director found the child in the classroom. This poses an immediate health, safety, and personal rights risk to persons in care. 8 Active Supervision Keeps Kids Safe - YouTube https://www.youtube.com/watch?v=vheeQM4DyzM and include elements in the training conducted. The Site Supervisor will submit the outline of training to LPA Anderson no later than COB on 03/04/2026 and signed attendance log once training is completed.
