Responsibility for Providing Care and Supervision: The licensee shall provide care and supervision as necessary to meet the children's needs. No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). 1 Create a zoning area for the staff and children. Reporting Requirements: Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours... 1 Director, reported the incident on 12/10/2025 and completed signed declaration indiciating that if they become aware of an allegations concerning a staff member they will report to the department. Any unusual incident... that threatens the physical or emotional health or safety of any child. This requirment was not met as evidence by... Based on interviews conducted the director did not report the termination of a former staff member who had inappropriate conduct with children outside of the facility. This poses a potential health and safety risk. 8
Personal Rights: Each child receiving services from a family child care home shall have certain rights that shall not be waived or abridged by the licensee regardless of consent or authorization from the child's authorized... To receive safe, healthful and comfortable accommodations, furnishings... 1 The director has already held an individual meeting with S1 and S4 and had them sign a copy to the addendum of the policy. The director provided LPAs with a copy of the addendum. This requirement was not met as evidence by: Based on interviews conducted it was disclosed by S1-S3 and C1-C2 that S1 and S4 were involved in a verbal altercation in front of children. This poses a potential health and safety risk to children in care. 8
Based on observations the licensee did not comply ,with the section cited above . During the tour LPA observed black, brown , yellow stains and holes on the carpet . Water dispencer had stains and dust . LPA observed couch cushions soiled , which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on records review and interview , the licensee did not comply with the section cited above . LPA reviewed 5 satff files and observed 3 staff members were missing records of immunization , which poses/posed a potential health, safety or personal rights risk to persons in care.
