Facility will submit proof to the department of safe, healthful and comfortable accomodations - (ie secured areas, removal of hazards/construction materials, repair outlet covers, removal of furnishings/toys identified as debris by

(a) The childcare center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement is not met as evidence by: Facility will submit proof of completion of indoor/outdoor renovations made including debris removal, and securing and making inaccessible areas still under renovation for the health and safety of children in care. Based on LPA observations and interviews conducted the facility did not comply with the section cited above in keeping building and grounds safe and in good repair. This is a potential risk to the health and safety of children in care. 8

Based on record review, the licensee did not comply with the section cited above in [4] out of [5] Mandated reporting certificates] [S1-S4] which poses a potential health, safety or personal rights risk to persons in care.
