This requirement was not met evidenced by: Per director, she is setting up play areas with more toys and equipment for the chlidren to minimize conflicts and eliminating the nap room from being used as the play area. (a) The licensee shall provide care and supervision as necessary to meet the children's needs. This requirement was not met evidenced by: 1 Per director, she will provide more training, pertaining to the topics of first aid procedures and incident documenting procedures and notifying director of injuries. She will email meeting agenda and staff sign in to LPA by 10/23/24. Child was not provided first aid for injury sustained and parent was not notified of injury. 8
(1) Twelve semester or quarter units; and at least six months of work experience...units...child growth and development; child, family and community, or child and family; and program/curriculum. This requirement was not met evidenced by: 1 Per Director, all age groups will conduct activities in the main room so that teacher with units can supervise aides working with children. Based on observation and record review, one of one teacher supervising children does not have child development units to be alone with children, which poses a health and safety risk to children in care. 8

Based on observation and video review, teaching staff did not notify director or parent of an injury a child sustained whil in care needing medical attention, which poses a health and safety risk to children in care. 8
(b) (1) On or before March 30, 2018, child care provider...employee of a licensed child day care facility shall complete the mandated reporter training ...shall complete renewal every two years...of the initial mandated reporter training. This requirement is not met evidenced by: 1 Per Director, she will have the 2 teaching staff complete the mandated reporter training via the free online link provided from LPA and email the certificate to LPA by 9/20/24. Based on observation, 2 teaching staff is missing the mandated reporter training certificate, which poses a health and safety risk to children in care. 8
(a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below. This requirement was not met evidenced by: 1 Per Director, she is hiring a fully qualified preschool teacher to start on Monday 8/19/24 and has verified that she has the required units. Director will send LPA all her information via email by Tuesday 8/20/24. Based on observation, 10 children were being supervised by a teacher's aide, which poses an immediate health, safety or personal rights risk to persons in care. 8

the fact that the facility agreed to provide transportation for Child#1, but the child was not taken to his school 1 The facility agreed to amend their admission agreement so that the transporation service provided by the facility is more specific, and transparent regarding the specific arrangements made with a parent of a child who wishes to be provided with these type of services. on one occasion, and was picked up late by the facility per the Director. This is a potential risk to children in care.
Any unusual behavior, any injury or signs of illness requiring assessment and/or administration of first aid by staff shall be reported to the child's authorized representative and recorded in the child's record. -This requirement is not met as evidenced 1 Licensee Davis said that she will reach out to PUSD regarding possible classes as well as having meetings with the parents about possible potential help. Licensee will tell LPA Bell a date and once a training meeting has been scheduled. by: Administration was aware of Child 1 touching/grabbing staff, and possibly kissing other children, but did not report it to the child’s parents, though it was documented on internal incident reports. *This poses an immediate health, safety and/or personal rights risk to the children in care.* 8
An incident on 02/08/22 when a child touched another child’s genitals was not reported to CCL. In reviewing the file, it was determined that the Center has not reported one Unusual Incident to CCL since they were licensed on 06/07/17.
Karishma Patel has Criminal Background Clearance but is not associated to Blooming Flowers CCC. A civil penalty of $500 has been assessed. *This poses a potential health, safety and/or personal rights risk to the children in care.*
Licensee Elaine Davis stated she will send LPA Bell the Director's packet by the close of business on 02/28/22. Director Jazmyne Thomas has been out since before March 2020 and though there is a Lead Teacher, the substituate director Antoinette Jordan still has not been designated as director. *This poses a potentil health, safety and/or personal rights risk to the children in care.*
LPA observed that children were not wearing face coverings, even while indoors. 1 Licensee Elaine Davis stated she will hand the parents and staff a letter and have them sign for it and placed the signed copies in each child's file as well as provide LPA Bell with a copy of the lettetr by the close of business on 02/28/22. *This poses a potentil health, safety and/or personal rights risk to the children in care.*
LIcensee Elaine Davis stated that she will associate Rebecca Meza to the license. She will email LPA the packet by COB on 02/28/22. Rebecca Meza is a licensee of a FCCH. Upon LPA's arrival, she was alone on the playground supervising six children. She is not associated to the facility, as verified on the Facility Personnel Report Summary. A civil penalty of $100 has been assessed. *This poses a potentil health, safety and/or personal rights risk to the children in care.*
