(a) The licensee shall provide care and supervision as necessary to meet the children's needs. Based on interviews, the facility did not comply with the section cited above, which poses an immediate health, safety or personal rights risk to children in care. 1 Facility Representative stated a staff is now stationed at the bathroom at all times. Facility Representative stated training on proper supervision. Facility Representative also stated procedures were updated to include disciplinary action if procedures were not followed.

(e) All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility:(1) Obtain a California clearance or a criminal record exemption as required by the Department Facility representative will not allow S4 or S5 to return to facility until they obtain criminal record clearance. The facility representative stated the facilities last day of operation will be 6/6/25. This requirement was not met as evidence by: Based on record review two staff (S4-S5) did not obtain a fingerprint clearance before working in the facility which poses an immediate health, safety or personal rights risk to children in care. 8
