Based on record review, the Lead Sampling Report in one(1)identified drinking fountain Room A Kitchen(R)had elevated lead above 5.5 ppb. This poses a potential risk to the health, safety, or personal rights of children in care. 8 Staff continue to serve the Children water and refilling their individual water container. Children have access to water.

Based on interviews, there was evidence of rodent(s) inside Classroom 22(State Preschool). This poses a potential health and safety risk to children in care.

A child was left unattended in the playground 1/30/2024. This poses an immediate risk to the health, safety, and personal rights to children in care. Upon receipt, Director shall post and provide copies of this licensing report, have LIC9224 signed and kept on file, to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. According to AB 633, parents must be provided with this report which contains this Type A deficiency for the next 12 months & copy of the signed acknowledgment form must be kept in each child's file.

Based on interview, the facility did not report change of director to Community Care Licensing (CCL) for review within 10 days of a change of director. This poses a potential risk to the health, safety, and personal rights of children in care.
