Director states results of retest or photos of remediated outlets will be submitted to LPA by 12/29/2022. This requirement is not met evidenced by record review of the lab report analysis from outlet Q, S, T, L, H, I, E, F, C, D, B, and G taken on 10/26/2022. Results indicated levels of lead in exceedance. This poses a potential risk to the health and safety of children in care.

(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement is not met evidenced by: 1 Director posted the lead sampling test results. The drinking fountain/outlet was closed off with plastic bag, taped and sign "Do not use". Director stated they contacted the school district to have a specific plan to fix the identified outlets/faucets and will retest and provide copies of the new test results to licensing office. Based on facility Lead sampling tests it was discovered that drinking fountain/outlet in Room 2, 5, 6, 7,9, 10, 12 had high level of lead. This poses a potential risk to the health of children in care. 8
