This requirement was not met as evidence by: 1 Director stated picnic tables were rearranged around the playyard to ensure supervision is being met on all areas of the play yard and they also had a staff meeting on 07/11/2024 regarding active supervision. LPA obtained a copy of staff sign in sheets for staff present for the 07/11/2024 meeting. Based on LPA’s interviews conducted, medical records and pictures obtained, it has been determined daycare child’s hair was pulled out by another daycare child in care due to lack of supervision. This poses an immediate health and safety risk to the children in care. 8
101223 Personal Rights (a)(3)To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or...This requirement was not met as evidence by: Based on LPA’s interviews Director stated a staff meeting was held on 07/11/2024 with staff regarding personal rights and a signature was required from staff as acknowledgment and understanding of children's perosnal rights. LPA obtained a copy of staff sign in sheets for staff present for the 07/11/2024 meeting and agenda. it has been determined staff yell at children and grab them from their arms. This poses an immediate health and safety risk to the children in care. 8
Based on observation and interview, the licensee did not comply with the section cited above. A bottle of carpet spot removal chemical was observed under the children's hand washing sink in classroom #4 and was accessible to the children, which poses an immediate health, safety or personal rights risk to persons in care.

(a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below. This requirement was not met: Director stated they have hired 3 additional teachers, out of the 3 staff that were hired 1 is an AM preschool opener. At 7:30am the children will go to their designated classrooms. Director stated they have done 1 on 1 training and modeling with staff. Director will submit POC by 4/18/24. Based on interviews conducted and records reviewed facility was out of ratio on 3/12/24, 3/13/24, and 3/14/24. This poses an immediate risk to the safety and health of the children in care. 8
(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure... (1) The licensee shall take measures to keep the center free of flies, other insects, and rodents. This requirement was not met: Director stated they will remind staff to make sure they continue to clean and sweep the floors to avoid from insects inside the classrooms. Director will submit written POC by 4/18/24. Based on interviews conducted S4 observed cockroaches inside the classroom about a month ago. This poses a potential risk to the safety and health of the children in care. 8
(e) Each licensee shall maintain an up-to-date list of qualified teacher substitutes... (1). Substitutes on this list shall be called immediately in case of emergency or illness to meet the teacher-child ratios required by this chapter. This requirement was not met as evidence by: Director stated they are opening an account with ChildCare Careers to be able to hire substitute staff in case of emergencies. Three staff have also been hired and planning to hire 1 more as a floater. Director will submit a written POC by 4/18/24. Based on records reviewed, facility did not have a substitute list for backup staff in case of an emergency in which, the facility did not have a preschool opener on 3/12/24, 3/13/24, and 3/14/24. This poses a potential health and safety risk to the children in care. 8
(e) As a condition of licensure, the areas around and under high climbing equipment, swings, slides and other similar equipment shall be cushioned with material that absorbs falls. (1) Sand, woodchips and pea gravel, or rubber mats... This requirement was not met: Director stated they will temperarly close off the 2 large slides to avoid children climbing and jumping of the slides. Director will place yellow caution tape to close slides. Director will submitted written plan of correction by the following day 11/16/23 to licensing office. Based on observation and interviews conducted, the artificial grass around the playground equipment is not a cushioned material that absorbs falls, making it unsafe for the children in care. This poses an immediate risk to the safety and health of the children in care. 8
Based on observation and interview per LPA's observation, it was observed that there was 1 teacher with 17 children. S1 was out of ratio for about 3 minutes. This poses an immediate safety risk for the children in care.

Based on observations and interviews during today’s inspection: 5 children disclosed that S1 yells at the children. Two staff diclosed they have observed S1 yell at children.
