101238 (a) (1) Buildings and Grounds (a)The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. (1) The licensee shall take measures to keep the center free of flies, other insects, and rodents. This requirement was not met evidenced by: 1 The Director stated they will provide a written preventative maintenance plan, remove the caterpillar remains from the outdoor toddler playground, and send documents to the department by 11/14/25. Director stated that it will encourage children to play opposite areas, with more activities. The playground structure, block area, and the steep stools will be sweep down, and designated one teacher around that area for redirecting the children. Based on observations and interviews with 4 staff members, LPA observed staff took the toddlers out to play in the Toddler playground where multiple caterpillars were observed crawling on the play structure, block area, couch and all over the ground.
