Based on observation, the outdoor play yard area that is shared between preschool and school-age children, does not have a shaded rest area for the children. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Based on review of video footage, Staff #1 was violating Child #1's personal rights by forcing the child to lay on the cot during nap time.
During inspection, LPA observed two small blue couches in room P3 that have several stains on them. LPA also observed two small brown couches in the Pre-K room where the upholstry is ripping on the arm rests. This prevents the furniture from being sanitary. The licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care.
