101238 Buildings and Grounds (a)(1) The child care center shall be clean, safe, sanitary ... at all times to ensure the safety and well-being of children, employees and visitors.The licensee shall take measures to keep the center free of... rodents. This requirement was not met as evidence by: 1 Executive director installed blockages at the bottom of classroom doors. Sticky rat traps were also observed to be in the facility. Facility also has contacted an exterminator to install metal mesh over the weekend to seal holes through the facility and instaling traps. Based on observation and interviews, a rodent has been observed to be at facility on multiple occasions. This poses a potential health, safety or personal rights risk to children in care. 8

Based on observation and file review, it was observed that one teachere was outside pushing a stroller "buggy" with 6 infants seated inside. This poses a potential health, safety or personal rights risk to children in care. 8
